At-HOME Fertility Medications

Depending on the woman’s medical needs, an oral fertility medication – typically Clomid and/or Letrozole – is prescribed to gently stimulate her ovaries into producing multiple eggs. Synarel is administered nasally for 3 days at the beginning of the cycle and then again 36 hours prior to surgical egg retrieval to trigger ovulation.

Today’s Assisted Reproductive Technology (ART) helps to increase the healthy birth rates of babies through genetic testing. This is because only the healthiest embryos are transferred for implantation during a fresh IVF cycle and/or Frozen Embryo Transfer (FET). Genetic testing helps to reduce the number of cycles required to achieve a healthy pregnancy.

Preimplantation Genetic Screening (PGS)

PGS is the testing of an embryo’s overall chromosomal normalcy prior to being transferred to the patient’s uterus in conjunction with IVF. PGS detects chromosomal deficiencies – aneuploidy – an extra or missing chromosome in an embryo.

Donor Eggs and Sperm

Regardless of the gender combination of LGBTQ couples – or singles wanting to be a solo parent – the process of selecting donor eggs or donor sperm has been substantially streamlined for our patients’ ease.

World class – yet affordable – support packages for the selection of donor eggs and donor sperm are offered. Our donor program is extremely successful because we use the most cutting-edge cryopreservation and IVF technologies.

IUI Procedure

The most common fertility treatment for lesbian couples is IUI. Timed with induced ovulation, the patient is artificially inseminated with a highly concentrated amount of motile donor sperm. The IUI procedure is performed in the doctor’s office and requires no anesthesia.

  • The patient’s monthly cycle is regulated using oral medication
  • An oral fertility medication – typically Clomid or Femara – is then self-administered by the patient to stimulate multiple egg follicle production
  • The growth of egg follicles are monitored through regular vaginal ultrasound imaging and blood tests
  • Once the egg follicles have reached a mature stage, ovulation is induced using an injectable trigger medication – hCG – human chorionic gonadotropin
  • The ovulation of multiple eggs, timed with sperm insemination, increases the patient’s chances of pregnancy
  • A highly concentrated amount of motile donor sperm is inserted into the cervix using a very thin catheter – similar to a pap smear procedure
  • The patient is instructed to self-administer progesterone to thicken her uterine lining in preparation for embryo implantation between the IUI procedure and the pregnancy test
  • A pregnancy test is scheduled

Two IUI inseminations are performed between 12 and 36 hours after hCG administration to increase the patient’s chances of pregnancy.

IVF Procedure

One partner’s eggs are harvested for IVF using donor sperm. The other partner carries the pregnancy.

  • The partners’ cycles are regulated to coincide with one another
  • The partner producing the eggs self-administers fertility medications to stimulate multiple egg production
  • The growth and development of egg follicles are regularly monitored with pelvic ultrasounds and blood level hormone measurements
  • Once the egg follicles have matured, the patient self-administers an ovulation inducing medication – hCG
  • The eggs are surgically retrieved using a transvaginal ultrasound-guided aspiration tool
  • The eggs are fertilized in vitro with donor sperm using The ovulation of multiple eggs, timed with insemination, increases the patient’s chances of pregnancy ICSI – a single sperm is injected directly into one egg
  • All resulting embryos will culture in a controlled laboratory setting until they reach an optimum stage of development – up to 5 days
  • A high quality embryo is transferred to the carrier partner’s uterus via an insemination catheter – similar to a Pap smear procedure
  • The carrier partner is instructed to self-administer progesterone to thicken her uterine lining in preparation for embryo implantation between transfer and the pregnancy test
  • A pregnancy test is scheduled

Gestational Surrogacy Using Donor Eggs

Gestational Surrogacy – Using donor eggs to be fertilized with the sperm from one of the partners. The resulting embryo is transferred to a surrogate who will carry the baby.

Gestational surrogacy may be used to achieve a pregnancy and birth using a surrogate  mother.

Look: One in six couples in America cannot conceive a biological child of their own. Frozen embryo adoption has successfully helped infertile couples build their families when other fertility treatments fail. Bottom Line: The success rate of IVF has led to an increase in the number of excess frozen embryos available to childless couples.

Adopt Frozen Embryos

Freezing excess, quality embryos after a fresh IVF cycle will increase a couple’s chances of conceiving in the future. This is not even the best part. FET Procedure

To begin the FET procedure, hormone medication is administered to suppress the woman’s pituitary gland and reduce the chances of unexpected ovulation. Estrogen acts to thicken and mature the uterine lining – like the natural estrogen produced by a developing egg follicle during a normal menstrual cycle.

During estrogen administration, monitoring of the uterine lining is required.

  • Transvaginal ultrasound for the doctor to evaluate the thickness of the uterine lining
  • Blood tests for the doctor to evaluate the level of estrogen in the blood

Double Benefit of FET: Monitoring for timing of transfer during a FET cycle is more flexible than in a fresh IVF cycle. The FET procedure is much less stressful on patients because the transfer date can be adjusted.

Progesterone is a medication administered in preparation for the FET to make the uterine lining receptive to the implantation of the embryo.

  • Progesterone matures the uterine lining
  • Progesterone creates a window of implantation in anticipation of a set FET date
  • Lupron injections are ceased upon the beginning of progesterone administration
  • Embryos are scheduled for thaw in the laboratory
  • FET is scheduled on a set date
  • Progesterone is continued daily to help prepare the patient’s uterine lining for implantation of the fertilized egg – the embryo
  • After the embryo transfer, a pregnancy test is scheduled on a date prescribed by the doctor through a blood draw

Healthy pregnancies have been achieved after an embryo has been frozen for up to 10 years. Pregnancy success through FETs is determined by the quality of the frozen embryo.

A fertility clinic in New York City has developed a form of in vitro fertilization that uses pills and a nasal spray, as well as urine and saliva tests instead of pricey injections and blood draws. Here’s what you need to know about needle-free IVF, or NF-IVF.

If you’re exploring IVF, you’ve learned about the bevy of side effects and steep cost of the conventional method. But what you might not have heard about just yet is a less invasive alternative called Needle-Free IVF, which is being touted as a groundbreaking technology offered exclusively by New Hope Fertility in New York City. Here’s what you need to know about the specifics and the cost of needle-free IVF.

The method was developed by John Zhang, MD, MSc, PhD, the founder/CEO of New Hope, in 2016. The unique method involves oral pills, vaginal suppositories, and nasal spray, but no injections, notes Zaher Merhi, MD, FACOG, the director of IVF research and development in IVF technologies at New York’s New Hope Fertility Center.

“Conventional IVF protocols have been used for over 35 years,” Dr. Merhi says. “It is about time to update IVF protocols to minimize risks and inconvenience related to conventional IVF. Conventional IVF involves daily injections and almost daily blood draws, which are too intense and heavy on patients’ body and mind. Injections are painful and expensive.”

To sidestep these uncomfortable aspects of traditional IVF, New Hope’s needle-free method involves oral and nasal fertility medications to more gently stimulate the ovaries into producing egg follicles. Because this process isn’t as rough on the body, patients are able to steer clear of conventional IVF side effects like weight gain—and get even higher-quality results, according to Dr. Merhi.

“If you think about pills and injections as food for the eggs, the less eggs there are, the less food they need,” Dr. Merhi explains. “Injections are strong food, so if you overfeed low number of eggs with injections, you will hurt their quality. A lot of studies have shown that less eggs retrieved due to minimal ovarian stimulation is correlated with better quality eggs.” This is especially true for women who are considered advanced maternal age (35+) and/or who have low ovarian reserve.

Like many conventional IVF patients, needle-free IVF patients start with a course of oral contraceptive tablets to prepare and stimulate the ovaries. But unlike conventional IVF, doctors will monitor the levels and baselines of pregnancy hormones (FSH, LH, estradiol, and progesterone) through urine and saliva, as opposed to daily blood draws. These tests “. . . are accurate for the hormone profiles that are needed for cycle monitoring, as long as the patient follows the instructions of the collection method,” Dr. Merhi says.

The patient then takes a “proprietary combination of nasal sprays” followed by a final spray for ovulation induction. Once a patient is cleared for retrieval, egg retrieval is done just as it would be with conventional IVF.

“Generally in the U.S., conventional IVF costs between $10,000-15,000,” Dr. Merhi says. By comparison, needle-free IVF will run between $5,000 and $7,000. The reason: “Conventional IVF involves taking injections which are very expensive,” according to Dr. Merhi.

Your chances of conception with needle-free IVF are “identical” to conventional IVF’s success rates, Dr. Merhi notes. That means that it varies from 40 to 65 percent in women under 35 and declines as women get older, down to less than 5 percent in women over 42.

Although needle-free IVF is suitable for any patient, it’s particularly useful for patients who:

The only factor that would disqualify a patient from being a candidate for needle-free IVF is an allergy to the oral medications or vaginal suppositories. “Otherwise, everyone qualifies,” Dr. Merhi says. “So far, the majority of patients who have gone through NF-IVF love it.”
